Who May Qualify
- Indiana resident enrolled in Medicaid
- Age 18 or older with a qualifying disability or chronic condition
- Assessed as needing a nursing facility level of care
- Able to live safely in a home or community setting with support
- Not currently enrolled in another Medicaid waiver program
